Document change log

Record of when documents on this site were added or had their version date updated, and what changed.

Keep this log current. Whenever the version date of any document on the site is updated, add a new entry at the top of DOCUMENT_LOG in this page (and update the matching file row above). Include:
  1. The date and time of the change (use Eastern Time).
  2. The document name and new version date.
  3. A detailed description of what changed — not only “updated,” but the substance (for example, a new template was added, a filename was corrected, or a requirement on the form changed).
